Medical Records

To request medical records of past hospital stays or surgeries at Val Verde Regional Medical Center (VVRMC) healthcare facility and to ensure compliance with the Health Insurance Portability and Accountability Act (HIPAA) privacy regulations, please download and fill out the Authorization to Release Protected Health Information form. Use the "My Health Portal" Proxy Access Request and Authorization Form as directed.

For patients in need of records from any of the VVRMC medical clinics, please refer to that clinic's location for details on how to obtain records.

You are now able to access any images taken at VVRMC by calling Medical Records at 830-778-3743 to request your study be uploaded. Click the link below to provide the information required to get your images. Please note the person requesting the images must be the one to enter the information.

How Does My Doctor Receive My Medical Record Information?

The physician's office may fax a request to the HIM department for lab, x-ray and items needed for your appointment. The request is processed directly to that office. The patient does not need to come to the facility. Ask the physician's office to request by fax.

International patients may bring a completed authorization for release form along with a letter of request in English from their doctor on letterhead including office fax number.

Information Needed for All Requests

Forms must be printed out, completed, signed by the patient and submitted along with a copy of the patient's driver's license.

For your convenience, there are several ways to submit your medical records request:

Other Uses

Insurance Purposes

The insurance company can mail a request to the facility and pay the copying fees.

Personal Use

An authorization for release form and a copy of your driver's license or valid international government-issued identification, such as a passport, can be sent/faxed to VVRMC. A fee will be incurred for this request.

Social Security Disability

Social Security typically requests your records by mail; this is then forwarded the HIM Department for processing. If the patient is requesting the records, there will be a fee.

Court

The same process will be followed as with a request for personal use. Remember to allow plenty of time to get the records by mail from VVRMC. A fee will be incurred for this request.

Attorney

Attorneys may send requests with required patient authorization to the facility, which will be forwarded to the HIM Department for processing. The attorney's office is responsible for the copying fees.

Records of a Family Member Who Has Passed Away

VVRMC will need a copy of the death certificate with your name listed as next of kin, authorization of release form, and copy of your driver's license or valid international government-issued identification (such as a passport). If you are submitting an international death certificate, it must be translated into English and notarized. A fee will be incurred for this request.
