Description:

Minimum Entry Level Qualifications: Ability to work with physicians in a collaborative manner. Three or more years of previous hospital experience in most aspects of health information management functions. Knowledge of JCAHO/CIHQ, HIPAA, federal and state regulations for healthcare facilities.

Academic Preparation: Bachelor’s Degree and RHIA certified/prepared

General Description:

Responsible for successful maintenance of patient records and organizational and administrative operation of the Health Information Management Department. Oversight responsibility for operational aspects of department. Serves as resource for facility regarding medical record content and regulatory requirements. Assures availability of the medical records at all times. Also oversees ongoing activities related to the development, implementation, maintenance of and adherence to the organization’s policies and procedures covering the privacy of and access to patient health information in compliance with federal and state laws and the healthcare facility’s information privacy practices.

Specific Responsibilities:

  • Possesses the ability to apply the principles of heath information management
  • Establishes, reviews and updates short-term and long-term goals for HIM Department in order to be consistent with hospital wide plans.
  • Implements and maintains ICD-10 coding and ensures compliance to regulations and payer needs.
  • Serves as point person for any RAC or other coding-related denials.
  • Develops comprehensive realistic annual budget that reflects departmental needs based on external and internal environments and the application of cost-effective management. Operates within this approved budget
  • Manages the department in a cost-effective manner by utilizing current materials management techniques, maximizing human resources and continuously reevaluating the manner in which services are delivered
  • Establishes policies and procedures necessary for effective and efficient systems and management of human resources. Assures compliance with all HIM policies and procedures.
  • Oversees staffing pattern and schedules to insure that they meet the Health Information Management Department needs.
  • Establishes and monitors standards of performance among all HM staff and ensures that standards are being consistently met.
  • Improves staff performance through ongoing counseling, coaching, delegation and feedback practices.
  • Assumes responsibility for a comprehensive departmental orientation program that is documented for each new employee.
  • Plans and coordinates staff development and continuing education.
  • Maintains knowledge and experience in information privacy laws, access, release of information and release control technologies.
  • Ensures the facility has and maintains appropriate privacy and confidentiality consent and authorization forms, disclosure tracking, information notices and materials reflecting current policies and procedures pertinent to privacy of protected health information (PHI).
  • Coordinates with practitioners in overseeing patients’ rights to inspect, amend, restrict access and obtain an accounting of disclosure of PHI when applicable.

  • Performs analysis of each discharged record, noting length of stay, service and would classification.
  • Understands how protected health information is used within a healthcare facility and how PHI is disclosed outside of a healthcare facility.
  • Prepares monthly statistical reports and distributes to specific departments, CFO, and administration by the 5th day of each month.
  • Oversees and is responsible for the completion of birth certificates and the release of information.
  • Works with all organizational areas involved with any aspect of release of protected health information to ensure coordination and cooperation with the facility’s policies, procedures and legal requirements.
  • Maintains current knowledge of applicable federal and state privacy laws (HIPAA) and accreditation standards.
  • Cooperates with applicable federal agencies or other legal entities and facility officers in compliance reviews or investigations.
  • Conducts department-wide meetings at least once a month, to maintain two-way communication, problem solving and information passing.

  • Attends meetings, prepares reports and analyzes and provides recommendations in a timely and accurate manner
  • Is responsible for maintaining performance improvement activities in the department.
  • Is responsible for educating staff on performance improvement activities
  • Represents the hospital in court cases involving subpoena of medical or clinical records
  • Enforces suspension policy for delinquent Health Information Management
  • Completes other duties as necessary and assigned by the CFO and/or administrator.

  • In coordination with appropriate departments and legal counsel, implements and monitors processes for receiving, documenting, tracking, investigating, taking action and reporting all complaints concerning the facility’s privacy policies and procedures.
  • In collaboration with Human Resources, the Information Security Officer, administration and legal counsel, ensures compliance with privacy practices and consistent application of sanctions for failure to comply with facility privacy practices for all individuals in the facility’s workforce, extended workforce and business associates.
  • Maintains current knowledge of applicable federal and state privacy laws (HIPAA) and accreditation standards.
