The primary functions of social services at Val Verde Regional Medical Center include admission screening of daily census reports to identify patients who may need an assessment of their social, economic and possible discharge planning needs.
High-risk screening involves reviewing with an interdisciplinary team those patients who—because of age, diagnosis, social or emotional factors, family or living situations, and financial status—may need early intervention and counseling, as well as locating appropriate community resources as indicated.
Secondary functions include linking patients and families with social and health networks that provide intensive counseling, referrals to other health care institutions for further medical treatment, and transportation and living accommodations when treatment is necessary in other locations. Information and referral about health care, financial resources and state and federal programs are also included.
The social service department is open from 8 a.m. to 4:30 p.m. Monday through Friday. The department is on-call 24 hours a day, including weekends.
